More than a year before the Virginia Tech shootings, Seung-Hui Cho appeared on the radar screen of state mental-health officials. He was ordered to get psychiatric help. But it is not clear whether he ever did.

It is also not clear who was ultimately responsible for making sure that Cho received help. State mental-health officials say they are determined to transform Virginia's mental-health system so that people like Cho do not fall through the cracks.

But they doubt that they will ever be able to predict or prevent someone having a similar experience in the future.
