NPR's Ina Jaffe reports that an issue from the early 1990s is back: A dozen states have pending legislation to expand health insurance coverage. And California, with seven million uninsured people, is leading the way with some of the most sweeping proposals. Analysts say the issue is once again reaching critical mass, with annual double-digit increases in premiums making health insurance a top concern of the middle class.
