For many years, doctors and hospitals in rural areas have complained that they receive lower payments from the federal Medicare program than their counterparts in big cities. Now, there's a move in Congress to address the gap. NPR's Howard Berkes reports on the situation in Clarke County, Ala., where doctors and hospitals are struggling.
