Human rights groups voice increased concern about civilian casualties in both insurgent attacks and U.S.-led counter-attacks. U.S. and Iraqi forces are gearing up for expected campaigns in the Sunni insurgent strongholds of Ramadi and Fallujah, where civilian casualties are expected to be high. NPR's Eric Westervelt reports.
