Wider Use Of Breast Cancer Radiation Technique Raises Concern

A debate is under way over a new technique for doing radiation treatment for early stage breast cancer. This approach is gaining popularity, but let's hear the full story here. Some experts worry that not enough research has been done to make sure it works as well as more established techniques. NPR's Rob Stein has more.

ROB STEIN, BYLINE: One of the places offering this new approach is at Holy Cross Hospital in Silver Spring, Maryland. That's where Lisa Galloway is getting treated for breast cancer.

LISA GALLOWAY: I'm Lisa Galloway. I am 53. I live in Leisure World, taking care of my mom as a caregiver.

STEIN: She was diagnosed in February. Her tumor was small enough that she could get it removed with a lumpectomy. But she still needed radiation to kill any cancer cells left behind. Her doctor suggested the new approach, called brachytherapy. Galloway jumped at the chance. It's a lot faster than traditional radiation.

GALLOWAY: Five days compared to 33 days - I was like, yay. So I wanted it so badly. And I got it. I got my wish.

STEIN: Galloway's doctor is Sheela Modin. She's a radiation specialist.

DR. SHEELA MODIN: I don't do everybody. It would only be someone who has early stage breast cancer.

STEIN: Modin knows some experts question whether this approach is being used too much, too soon. But she's confident it's a good option for patients like Lisa Galloway.

MODIN: Ms. Galloway?


MODIN: I'm just going to go over everything with you. Is that ok?


MODIN: So, you know, you're lying here. And what we're going to do...

STEIN: This is Galloway's first day of brachytherapy. To get ready for the treatment, a surgeon temporarily implanted a small device in the spot where her tumor was taken out. The device has a bundle of tiny, flexible tubes, and the ends of those tubes are coming out of her breast. A technician connects each of the tubes from the implant to a radiation machine. Then Modin and the technician walk out of the room to a bank of computers. That's where they monitor the patient and control everything.

MODIN: And now she's going to actually start the treatment.


STEIN: This set-up lets Modin deliver very high doses of radiation to very specific spots inside Lisa Galloway's breast. The radiation comes from a tiny pellet that's at the end of a very thin wire. One by one, the radiation-tipped wire snakes in and out of each tube inside Lisa Galloway's implant.

MODIN: So we're on catheter seven. And as you can see, it's going pretty quickly.


STEIN: Sounds like a video game.

MODIN: Yeah.

STEIN: The wire goes in and out of the next tube, and the next. After only a few minutes, it's all over.

MODIN: And we're almost done. And we're done now.

STEIN: Lisa Galloway will go through this 10 times total - twice a day for five days. And that's it.

Galloway is far from alone. In recent years, the popularity of brachytherapy for breast cancer has soared. That worries Bhadrasain Vikram of the National Cancer Institute.

DR BHADRASAIN VIKRAM: The use went from less than 1 percent of patients in 2001 to 10 percent in 2006. So it was a 10-fold increase over a five-year period.

STEIN: He says there's just not enough proof yet that brachytherapy works well enough.

VIKRAM: I see that the rush to brachytherapy is somewhat inappropriate, because it hasn't been yet proven in a randomized trial to be as effective as a standard treatment.

STEIN: And, he says, there are some big concerns.

VIKRAM: That the tumor will recur and women will need more mastectomies and/or the tumor might spread to other parts of the body and kill the woman, or it may have more toxicity in the long term.

STEIN: Those fears spiked in December when Benjamin Smith of the M.D. Anderson Cancer Center in Texas unveiled the results of a big study.

DR. BENJAMIN SMITH: We found that the decision of whether or not a patient was treated with brachytherapy or whole breast irradiation was the single most important predictor of whether or not they had a mastectomy within five years of their cancer diagnosis.

STEIN: Now mastectomies were rare no matter what kind of radiation women got. But they were about twice as common among the women who got brachytherapy. Smith says that's a red flag that brachytherapy might not be snuffing out cancer as well.

SMITH: The most plausible explanation for our data is that women treated with brachytherapy were at an increased risk of having a recurrence of cancer in their breast.

STEIN: Smith and his colleagues also found more minor complications, such as infections and bleeding.

SMITH: When you put together a significantly increased risk of a lot of different complications and a treatment that's slightly less effective potentially than whole breast irradiation, then you start to wonder, what is the role of this treatment? And have we adopted it too quickly before we really understand how to use it correctly?

STEIN: Smith and Vikram say women should only get brachytherapy for breast cancer as part of carefully designed studies. But other doctors disagree. For one thing, they say the data Smith used are old. They come from when doctors were just starting to learn how to do brachytherapy for breast cancer.

Douglas Arthur of the American Brachytherapy Society argues there's plenty of other evidence that it works well - at least for some patients, mostly older women with very small tumors that haven't spread.

DR. DOUGLAS ARTHUR: We have data from several institutions that shows that in appropriately selected patients that are treated with five-day treatment, the outcomes are very good and the toxicity is very low.

STEIN: In addition to being more convenient, the more targeted approach avoids some complications.

Martin Keisch is a radiation oncologist at the University of Miami Hospital.

DR. MARTIN KEISCH: Less radiation means less side effects. They don't get fatigue. They don't get the darkening, reddening, tenderness of the skin related to external beam radiation. You don't get that with the internal radiation that you are receiving with brachytherapy.

STEIN: Back at Holy Cross Hospital, Dr. Sheela Modin acknowledges that there are still questions about brachytherapy.

MODIN: At this point we're being very conservative, based on the data that's available right now. And in select patients, and they tend to be older women, that this is a reasonable approach.

STEIN: Lisa Galloway is just relieved to have one day of radiation down.

GALLOWAY: Four more days. I just say four more days and that's it.

STEIN: She isn't crazy about walking around with the brachytherapy implant. And she says it's kind of weird to feel little jolts of vibration during the treatment. But she's up, and heading home minutes after it was all over.

GALLOWAY: I'm very happy, yeah, very happy.

STEIN: After her final day of treatment, she can get the implant taken out and start to put all of this behind her. Meanwhile, the debate over brachytherapy continues. A big government study is trying to clarify where the treatment stands but the results won't be out for years.
